Information
In order to have your child/ren registered for the SY 2009-2010, you should do the following things:
New Students
New students will have to fill out the following forms:
- SD Form 600
- DSE 803
- DSE Form 910-R (explanation of codes is located on the back of the form) International service members fall into Category 3G or 3P.
- DoDEA Form 600A (Race/Ethnicity and Home Language Questionnaire)
- DSA 105 (Request for Student Record)
- Computer Access and Internet Policy (1 per child)
- Publication Permission Form
Print off one Health Assessment per child and make an appointment with a healthcare provider to have a physical and update the student's immunizations if needed. NOTE: All forms must be completed and turned in prior to the beginning of the school year, especially if you want your child to be able to participate in sports!
Important documents to bring with you at the time of registration
- Sponsor's valid U.S. / SHAPE ID card
- Confirmation of status form.
- Documents listed on DSE Form 910-R
- 8.00 Euro per child for insurance (mandatory by Belgian law)
- Proof of child(ren)'s birth date (passport or birth certificate)
- Child(ren)'s Immunization Record
Report Cards/School Records from schools attended prior to SHAPE High School.

International Students
International students and their parents should make arrangements for ESL testing with the ESL teacher/coordinator at the time of registration.
After the tests, students and parents will make an appointment with the Guidance Counselor at least two days after the ESL test so the results will be available. The student must be present for the appointment
Students in grades 10-12 need to bring the following documents to the Guidance Counselor appointment:
- Certificate of Promotion to the next grade
- Grade Report indicating grades (marks) in each subject taken during grades 9, 10, and 11
- Hours of study in each subject taken in grades 9. 10, 11
- English translations can be done through the student's NMR
IMPORTANT!
All listed forms must be turned-in at the time of registration except for school physical form. School physical form must be turned-in after the physical has been completed, no later than first two weeks of school. Non-presentation of the above listed forms will preclude registration. Children may only be registered by parents or legal guardians and forms must be signed by the sponsor or the spouse.
